Reema Health partners with health plans to engage hard-to-reach members by addressing social determinants of health and behavioral health needs through high-touch, community-based engagement and social needs navigation. Reema employs local Community Guides, individuals from the same communities as the members, to build trust and deliver personalized support around housing, food, transportation, and mental health.
